You can get a copy of the Application for Readmission form one of three ways: - Fill out a form at the CLA Student Information Office, 49 Johnston
- Download an Application for Readmission here, from Forms Online
- Call (612) 625-2020 to have one mailed to you.
You can submit your completed application one of three ways: - To the CLA Student Information Office, 49 Johnston Hall for immediate processing (CLA graduates should allow 2-3 days for processing).
- By fax (612) 626-0022. Allow 3-5 days for processing.
