Applying for readmission

You can get a copy of the Application for Readmission form one of three ways:
  • Fill out a form at the CLA Student Information Office, 49 Johnston
  • Download an Application for Readmission here, from Forms Online
  • Call (612) 625-2020 to have one mailed to you.
You can submit your completed application one of three ways:
  • To the CLA Student Information Office, 49 Johnston Hall for immediate processing (CLA graduates should allow 2-3 days for processing).
  • By fax (612) 626-0022. Allow 3-5 days for processing.
  • By mail to: CLA Student Information Office 49 Johnston Hall 101 Pleasant St SE Minneapolis, MN 55455 Allow 5-10 working days for processing.

If you have taken courses at another institution during your absence, have an official transcript sent to:
